Inspection, Testing and Certification per NFPA 80 Standards

National Fire Protection Association (NFPA) standards require building owners to have their rolling and sliding fire doors inspected and tested annually, and to maintain written documentation of such inspections. CityWide Door ’s Fire Door Drop Testing (FDDT) Program helps building owners and managers stay in compliance with NFPA 80 standards, while maintaining the proper operation and full closure of all facility rolling and sliding fire doors.

Fire Door Drop Testing Services Include:

	The inspection, testing and certification of your facility’s rolling and sliding fire doors to meet NFPA 80 Section 15-2.4.3-5 requirements by trained and certified technicians.
	A written Fire Door Inspection and Drop Test Report, which documents each FDDT service call for your facility’s records along with recommendations for any needed repairs or product replacement.
	A “Fire Door Certification” tag is attached to each serviced fire door indicating the date the door was inspected, tested and certified
	Copies of your facility’s Fire Door Inspection and Drop Test Reports are kept in our Company records in case of an emergency or any compliance questions.
	You will be contacted on an annual basis to schedule future FDDT service appointments to help keep your fire doors in compliance with NFPA 80 Section 15-2.4.3-5 and in good working order.

Defective, damaged or non- functional fire doors can lead to tragic results and may subject building owners and managers to catastrophic losses and increased legal liability. CityWide Door ’s FDDT Program provides for your facility’s ongoing compliance with NFPA 80 Section 15-2.4.3-5 requirements, while establishing and maintaining FDDT records for your facility on an annual basis.

Fire Door Drop Testing Program for Rolling and Sliding Fire Doors Terms & Conditions CityWide Door's Fire Door Drop Testing (FDDT) Program provides FDDT service for all rolling and sliding fire doors on an annual basis as requested by the member upon enrollment into the Program. Term FDDT Program membership, and the benefits provided herein, shall be in effect for a period of one year from the date the first FDDT service is completed (actual FDDT service work performed). Renewal FDDT Program membership renews automatically for a period of one year from the date each FDDT service is completed. Scheduling of FDDT Service Approximately one to two weeks before FDDT service is to be performed (per member's Enrollment Form) a representative from the Company will call the member and/or their on-site contact to schedule a date for FDDT service. Method of Payment Payment for FDDT services shall be made at the time the actual FDDT service is performed, unless the member has made other billing arrangements with the Company. Limitations Under this FDDT Program a member is entitled only to regularly scheduled FDDT service on listed rolling and sliding fire doors, and related FDDT Program member benefits, as described herein. Scope of FDDT Service FDDT service provides for a comprehensive inspection, testing and certification of the rolling and sliding fire doors listed on the member's Enrollment Form. A Fire Door Inspection and Drop Test Report will be provided to the member upon completion of each door's inspection, including notation of why a tested door failed to pass the inspection/drop test. Any authorized repair and/or replacement work performed to bring a fire door to NFPA 80 Section 15-2.4.3-5 compliance will be at the member's additional expense (separate from standard FDDT Program service charges). This FDDT Program is not a maintenance contract, and the Company has no obligations beyond those stated herein. Liability The inspection and tests conducted by CityWide Door under this FDDT program are performed solely to determine NFPA 80 Section 15-2.4.3-5 compliance, as of the date of FDDT service, as documented on the Company's Fire Door Inspection and Drop Test Report (the report) which accompanies each FDDT service call. The Company does not guarantee operation of the fire doors inspected and tested nor their compliance with the standards set out in the report in the future. The Company shall not under any circumstances be liable by reason of such inspection and tests, directly or indirectly: for any accident, breakage or damage to the doors or any machinery, appliances or property connected therewith; for any injury to any person or persons resulting from the use or operation of the doors, however caused; nor for consequential damages of any kind. Force Majeur The Company shall not be responsible for any delay or failure to render FDDT services hereunder caused by Acts of God, governmental actions or regulations, strikes or other labor troubles, or any other causes that are beyond the control of the Company.
